HMO, PPO, EPO or POS: what the letters mean for you.

The plan type decides which doctors you can see, whether you need referrals and what happens out of network. Knowing the difference helps you avoid surprise bills.

HMO: stay in network

An HMO usually covers care only from doctors who work for or contract with the HMO, and generally won't cover out-of-network care except in an emergency. HealthCare.gov

PPO: more choice, higher cost outside

A PPO costs less when you use in-network providers. You can see providers outside the network without a referral, but you pay more. HealthCare.gov

EPO: network only, no referral maze

An EPO covers services only from in-network doctors, specialists and hospitals, except in an emergency. HealthCare.gov

POS: a primary doctor runs point

A POS plan costs less in network and requires a referral from your primary care doctor to see a specialist. HealthCare.gov

Which plan type is cheapest?

It depends on the plans in your county. HMOs and EPOs often have lower premiums because they limit the network, but compare the premium together with the deductible, copays and out-of-pocket maximum.

Does a PPO cover any doctor?

No. A PPO covers out-of-network care at a higher cost, and the provider may bill you for the difference. Check the plan documents before you rely on out-of-network care.

How do I check my doctor is in network?

Use the plan's current provider directory for the exact plan, and call the office to confirm. Directories can change during the year.
